This chapter examines several ways of managing “last chance” experimental therapies, which have often been the focus of litigation and legislative mandates. Drawing on results from a three-year research project involving collaboration with a number of leading managed care organizations, it reports on some very promising exemplary practices observed for managing last-chance therapies. Each of these practices illustrates accountability for reasonableness.
